Survey on Hepatitis C and Diet

People with hepatitis C are invited to take a few minutes to participate in the following survey.

We are collecting information from people living with hepatitis C which will ultimately help us to develop effective resources and improve our services. The 15 questions below this introduction take around 5-10 minutes to complete.

The aim of this research is to understand how people with hepatitis C can be supported to enjoy a healthy diet and maintain good health. One of our aims is to identify barriers that prevent people from enjoying a healthy diet and then develop a resource (for example a cookbook) or other strategies that make healthy eating easier.

THE SURVEY IS COMPLETELY ANONYMOUS - no names are used. We cannot identify you in any way personally. All questions are OPTIONAL. Some questions are tick box and some provide space for comments.
